Levels of Care Include:

General Inpatient Care

Short-term hospice inpatient care for patients close to death and in need of skilled care for pain management or symptom management.

Respite Care

Five-day respite care for Community Hospice patients whose caregivers need a short break.

All services provided at the Care Center are under the direction of the Community Hospice Medical Director. Registered nurses are on duty 24-hours a day, providing skilled nursing care. Other services provided include personal care, dietary, social work, grief counseling, chaplain services and trained volunteers.

Additional Services Provided:

  • Medications, medical equipment and supplies

  • Special dietary needs addressed with professional skill, sensitive to personal preferences

  • 24-hour family, friend and pet visitation

  • Grief counseling and bereavement support services for patients, families, adults and children

  • Therapy programs include music and pet therapy
