Levels of Care
Hospice care is designed to meet the changing needs of patients and families throughout their journey. Medicare defines four levels of hospice care to ensure patients receive the right support, comfort, and symptom management at the right time. Whether care is provided at home, during a short-term inpatient stay, or during times of increased medical need, our hospice team is here to provide compassionate care every step of the way.

Hospice Levels of Care
Hospice care is designed to meet patients wherever they are in their journey. Because every patient’s needs are different, hospice provides several levels of care to ensure comfort, support, and peace of mind for both patients and families.
Our team works closely with patients, caregivers, and physicians to determine the appropriate level of care and adjust services as needs change.
Routine Home Care
Routine Home Care is the most common level of hospice care. Patients receive hospice services wherever they call home including private residences, nursing facilities, retirement communities, or other long-term care facilities.
This level of care focuses on comfort, symptom management, emotional support, and improving quality of life while allowing patients to remain in familiar surroundings.
Services may include:
- Regular visits from nurses
- Hospice aide assistance
- Social work and spiritual care support
- Volunteer Services
- Medication and symptom management
- Bereavement support for families
Our Hospice Team is available 24 hours a day, 7 days a week for support and urgent needs.
Crisis Home Care
Continuous Home Care, sometimes called Crisis Care, is provided during periods of increased medical need when a patient is experiencing uncontrolled symptoms or a medical crisis.
The goal of this level of care is to help manage symptoms and keep the patient comfortable at home whenever possible.
During Continuous Home Care, nurses and hospice staff may provide extended bedside care for a period of time to address issues such as:
- Severe pain
- Dificulty breathing
- Anxiety or agitation
- Uncontrolled nausea or other acute symptoms
Once symptoms are stabilized, the patient typically returns to Routine Home Care.
General Inpatient Care
General Inpatient Care is short-term care provided in a hospice inpatient unit, hospital, or skilled nursing facility when symptoms can no longer be effectively managed in the home setting.
This level of care is focused on intensive symptom management and comfort during periods of severe pain or complex medical needs.
Patients may receive inpatient care for symptoms such as:
- Uncontrolled pain
- Severe breathing difficulties
- Persistent nausea or vomitting
- Complex symptom management requiring close monitoring
When symptoms become manageable, patients often return home or to their previous place of residence with Routine Home Care services.
Respite Care
Caring for a loved one can be physically and emotionally exhausting. Respite Care provides short-term inpatient care to give family caregivers an opportunity to rest and recharge.
During respite care, the patient receives around-the-clock care in a Medicare-approved facility while caregivers take a temporary break. Respite stays are typically short-term and may last up to five days at a time.
This level of care supports not only the patient, but also the well being of the caregivers who are providing daily support.
Care That Adjusts With Your Needs
A patient’s hospice needs may change over time, and hospice levels of care can change as those needs evolve. Our team continually evaluates each patient’s comfort and support needs to ensure the right care is provided at the right time.
If you have questions about hospice services or levels of care, our team is here to help guide you.
