Care for those facing life-limiting illnesses

The goal of hospice is not to cure illness, but to ease a patient’s passage through the final stages of life with dignity, comfort, and compassion. Community Hospice helps patients and their families navigate healthcare decisions alongside their physicians and hospice professionals, ensuring care aligns with the patient’s wishes and goals.

At Community Hospice, we care for the whole person—physically, emotionally, and spiritually—while also supporting the family members and loved ones walking alongside them. Our compassionate team works together to manage pain and symptoms, provide guidance and encouragement, and create meaningful moments of comfort, peace, and connection.

Hospice care is not about giving up hope—it is about redefining hope. Rather than focusing on cure, hospice focuses on comfort, quality of life, and making the most of the time that matters most. Care is provided wherever the patient calls home, whether in a private residence, nursing facility, retirement community, or hospice inpatient center.

You Are Never Alone

Our interdisciplinary team may include physicians, nurses, aides, social workers, chaplains, bereavement counselors, and trained volunteers who work together to provide personalized care tailored to each patient and family’s unique needs. By caring for the physical, emotional, and spiritual needs of both patients and loved ones, we strive to bring comfort and support during life’s most meaningful moments.

Through every step of the journey, Community Hospice is here to provide compassionate support, expert care, and reassurance when it is needed most. We are honored to walk alongside patients and families with dignity, comfort, and understanding.

Enhancing the Quality of Life of Patients

The patient, family, and hospice interdisciplinary team work together to develop a personalized plan of care focused on comfort, support, and quality of life. This comprehensive care plan is tailored to each patient’s individual needs and goals, allowing care to be provided in the setting that feels most comfortable and familiar whenever possible. Members of the Community Hospice interdisciplinary team may include:

    • Community Hospice Medical Director and Associate Physicians: oversee patient care and are available for consultation with all of our patients. The Medical Director and Associate Physicians also provide care for patients who are without their own physician.
    • Personal Physician: your own primary care or attending physician who oversees your plan of care.
    • Hospice Nurse: makes regularly scheduled visits and coordinates the plan of care with other team members.

    • Pharmacist: assists with medication management.

    • Social Workers: help with the psychological and social aspects of a terminal illness and grief.

    • Spiritual Care Counselors: available to provide spiritual support for our patients and families.

    • Bereavement Counselors: available to provide pre-bereavement and grief support.

    • Certified Nursing Assistants: help you with personal care.

    • Specially Trained Volunteers: your helping hands and friendly listeners.
