Taking the first step toward hospice care can feel overwhelming, but you do not have to navigate it alone. At Community Hospice, we are here to answer your questions, provide guidance, and help you understand your options with compassion and care. As an independent, nonprofit hospice organization, we proudly serve patients and families in Boyd, Carter, Elliott, Greenup, Johnson, Lawrence, and Martin counties in Kentucky, as well as Lawrence and Scioto counties in Ohio. The First Step in Making a Referral

Anyone can make a referral to hospice care whether it is the patient, a family member, a friend, or a healthcare professional. A physician referral is not required to begin the conversation. Once we receive a referral, our compassionate team works closely with the patient and family to gather the necessary medical information, determine eligibility for hospice services, and schedule a visit with our admissions team. We are here to guide you through each step with care, support, and understanding.
